If you are new client, fill out an application for assistancehere. After submitting your application, refer to the email address used for submission to create a login for an appointment.
If you are a returning client, log in to your client portal here. After logging in, select the appointment type, day, and time of your desired appointment.

You will receive confirmation text and email after your appointment has been successfully booked.
If you are a new client, please log in to your client portal to finish filling out additional forms and upload any documentation needed 72 hours BEFORE your appointment.

We understand that things happen, and sometimes you cannot make it. Please cancel your appointment by emailing clientinfo@streetwisegeorgia.org or visiting our Contact Us page 48 hours before your appointment time. Please include your full name, email address, and appointment date to avoid a No Show penalty.

To reschedule your appointment, please make a new appointment in your client portal. We will cancel your previous appointment for you.

If you are a returning client, you can book your appointment every 6 weeks. If you would like to book a Saturday Mobile Food Pantry, your last weekday appointment cannot be 2 weeks before or after that Saturday.

If you are having trouble getting ahold of us by phone due to busy lines, please try making an appointment here online.

If you did not cancel your appointment using one of the 3 ways above 48 hours before your appointment date and time, you will lose all services for 12 weeks, that includes two weekday and one Mobile Food Pantry appointment.

If you missed your appointment without cancelling 48 hours before, you will receive an email and letter notifying you that you will not be able to receive services for 12 weeks.

If you are a new client, please bring:
Photo ID and Social Security card for yourself and your spouse
Social Security card OR Photo ID for EVERYONE over 18 years old in the household
Date of birth/ages/relationship to you for ALL people in the household
Proof of Home address: Electric, Gas, Water Bill, or Rental/Lease Agreement
Proof of income for the entire household: Pay Stubs, Award Letters, Bank Statements
List of monthly bills for the entire household (medications, utilities, insurance, car payments, etc.) This is to help you complete your application.
If you are a returning client, please bring any documentation that was required at your last appointment. If you do not provide missing information after 2 appointments, you will not be allowed services until the information is received at StreetWise in advance.

Weekday appointments may be made every 6 weeks. Aside from food, you can receive support from our Client Advisors, visit Cedar’s Closet for clothing, and Heaven’s Grocery Store for essentials such as baby supplies and hygiene products.
Mobile Food Pantry is every second Saturday or third (depending on the holidays). This is a drive-through service providing food only while staying in your car. Our volunteers will be there to load your trunk.
